If this is a new report, or if you selected "Edit Data" from the Report Options screen, the Report Data screen comes up. If this is a new report, you must fill in the fields on this screen before you can type the report. Data from this screen will be used in the headers and footers of the printed output of the report and for future searching of the report.
Note 1- The fields on this screen may change depending on the form that you have selected.
Note 2- Certain fields are required and must be filled in before you can press the F10 key. Other fields are optional and may be left blank. All required fields will have their descriptions shown in bold face. If you don't know some of the values at this time, the Report Data screen can be displayed and edited from within the Word Processing screen by pressing F2.

| 1. Enter a value for each field. Names must be in LAST, FIRST format (with a space after the comma). Dates must be in MM/DD/YYYY format. All times must be in military time in HH:MM:SS format (for example, "17:30:00" means 5:30pm). |
| 2. Pressing ENTER or the Down Arrow will advance to the next field. |
| 3. Fields that require codes to be entered may have lookup screens associated with them. A field that has a lookup screen will have an "(F1)" at the end of its description. Pressing F1 will cause a lookup screen to come up, allowing you to select a value from a list. Note- Some fields are set up to have their lookup screen automatically appear when you enter the field. |
| 4. When finished with entering all report data fields, press the F10 key to signal that you are finished. The Word Processing screen will then be displayed. |
| 1. | When on a date field, pressing the PGDN key will decrease the date by 1 day. Pressing PGUP increases the date by one day. |
| 2. | To automatically insert a provider's (e.g. doctor) name into a field, press CTRL-D. A lookup screen will appear, allowing you to search for a provider to insert. |
