Bracketed field shows blanks

Usually due to a syntax error somewhere in the bracketed field.  Make sure that you do not have any blanks between the various parts of the bracketed field.  For example, "[r.  ptname      ]" will not work because there one or more blanks before the "ptname".

Bracketed field wraps to next line

Usually due to the length of the actual date that is merged in takes up more space than the bracketed field does in the form.  Try replacing the ending bracket ( "]" ), with a right parenthesis ( ")" ) so that trailing blanks will be trimmed from the field.

Pages do not break properly when Automatic Orphan Control is on

Make sure that there are no manual page breaks located down by the signature block.

Lines of text are missing or duplicated at the bottom or top of the page

Usually due to the top or bottom margin not being large enough to hold the header or footer.  Go into Forms Maintenance and check to make sure that the top margin is large enough to fit the Header (plus 1/4 inch) and that the bottom margin is large enough to fit the Page Footer (plus 1/4 inch).
